Job Overview:
We are seeking Enrollment Case Managers to facilitate the eligibility determination process for HCBS (Home and Community-Based Services) applicants. This role involves conducting intake visits at applicants' homes, providing assistance throughout the enrollment process, and maintaining relationships with community organizations and service providers.
- Travel within the assigned county to conduct intake visits for HCBS applicants.
- Facilitate the eligibility determination process and provide assistance with HCBS program enrollment.
- Help applicants with the Compass application, MCO, Life Provider, and Supports Coordinator selection.
- Collaborate with community-based organizations, physicians, County Assistance Offices, and other service providers.
- Ensure efficient processing of applications, providing extra support for stalled cases.
- Maintain accurate records and report on schedule availability.
- Attend regular meetings and assist with other duties as assigned.- 1+ year of experience in public or private social work. - 2+ years of casework experience, including experience assessing functional abilities for community-based services.
- Bachelor's degree in social welfare, sociology, psychology, gerontology, or a related field (or equivalent experience and training).
- Travel within assigned county, occasional travel to neighboring counties.
- Training will be conducted virtually through MS Teams.
- Gas reimbursement provided for travel outside the assigned county.
