Excelsior-Integrated Care Center is currently accepting applications for a Care Coordinator. The Care Coordinator supports Transition Age Youth (ages 17–24) as they transition toward housing stability and increased self-sufficiency. Working as part of a multidisciplinary team, this role provides care coordination, housing support, and life skill development to ensure continuity of care and successful community integration. The Care Coordinator serves as a liaison between the youth, service providers, and housing systems to reduce barriers and promote long-term stability.
Qualifications:
•BA in counseling, social work, or a related field is preferred. HS or GED required.
•Ability to be registered as an Agency Affiliated Counselor Credential through the Washington State Department of Health within 60 days.
•Minimum three years of experience working with youth or young adults, housing, or behavioral health
•Comprehensive knowledge of community resources and housing systems.
•Excellent communication, organizational, and interpersonal skills.
•Must be 21 years or older.
•Ability to pass a background check.
•Valid driver's license and acceptable driving record in accordance with organizational requirements
Responsibilities:
•Build trusting relationships with youth using trauma-informed practices.
•Assess housing needs and develop screening and tracking tools, individualized housing plans, and life-long goal planning.
•Support applications for transitional or permanent housing.
•Assist with move-ins, lease reviews, and housing navigation.
•Coach individuals in housing-related life skills such as budgeting, roommate communication, and apartment care.
•Monitor housing stability and respond proactively to risk factors.
•Coordinate with landlords, housing authorities, and the internal team to maintain housing.
•Schedule and support the intake process and return to community planning.
•Develop and maintain individualized care plans across health, housing, and social domains.
•Facilitate communication and coordination among the youth’s care team.
•Provide coaching in transportation, budgeting, cooking, hygiene, and other life skills.
•Encourage engagement in services such as therapy, employment, education, and peer support.
•Provide transportation for youth as needed.
•Attend and actively participate in required meetings and trainings.
•Maintain confidentiality and professionalism in all interactions.
•Evening or Weekend work may be required on occasion to meet client support needs.
