Overview
ECMs primarily work in applicants' places of residence, conducting intake visits and assisting with the eligibility determination for HCBS programs. Travel within the assigned county and neighboring areas is required.
- Travel to applicants' residences for intake visits and facilitate eligibility determination
- Assist applicants with HCBS applications, including data entry and selecting MCOs, Life Providers, or Support Coordinators
- Advise applicants on their rights and responsibilities regarding HCBS programs
- Develop relationships with community-based organizations and stakeholders to improve the application process
- Foster partnerships with physicians, County Assistance Offices, providers, nursing homes, and rehabilitation facilities
- Provide additional support to stalled cases and assist with maintaining the efficiency of the application process
- Attend meetings and report on schedule availability
- Occasionally lift/carry items up to 25 pounds
- Perform other duties as assigned
- Minimum 1 year of experience in public or private social work
- Minimum 2 years of casework experience, including assessing individuals' functional needs
- Bachelor’s degree with at least 12 semester hours in sociology, social welfare, psychology, gerontology, or related social science, or an equivalent combination of experience and training
- $0.70/mile travel reimbursement
- Weekly salary and gas bill payment
