Our interdisciplinary team brings compassionate, innovative care to homebound patients and those in senior care communities. We are expanding and looking for experienced, self-motivated Wound Care Nurses to join our mobile wound care team!

Position Summary:

As a Mobile Wound Care Nurse, you’ll deliver high-quality wound care to patients in their homes, assisted living facilities, or skilled nursing facilities. This is both a clinical and relationship-driven role, requiring independence, professionalism, and excellent communication skills.

Responsibilities:

Travel to assigned patient locations to perform wound assessments and treatments

Document visits in a timely and accurate manner using our EMR system

Collaborate with medical directors, physicians, and partner providers (Home Health, SNFs, ALFs, etc.)

Educate patients and caregivers on wound care management and prevention

Maintain positive relationships with referral partners and facility staff

Qualifications:

Current RN or LVN license in the state of Texas

At least 1 year of wound care experience (WCC or other certification preferred, but not required)

Reliable vehicle and valid driver’s license

Strong organizational and time-management skills

Ability to work independently and with minimal supervision

Excellent communication and interpersonal skills
