Job Summary
The LPN Admissions Coordinator plays a vital role in the healthcare team, responsible for overseeing the admissions process for new patients. Responsibilities
•Conduct comprehensive assessments of potential patients to determine eligibility for admission.
•Collaborate with interdisciplinary teams to develop individualized care plans.
•Provide crisis intervention and management as needed during the admissions process.
•Educate patients and families about available services, treatment options, and resources.
•Maintain accurate documentation of patient information and admission processes in compliance with regulatory standards.
•Supervise support staff involved in the admissions process to ensure quality care.
•Utilize motivational interviewing techniques to engage patients effectively.
•Assist with workforce development initiatives related to patient admissions.
•Stay informed about Medicare regulations and Section 8 housing options relevant to patient needs.
Qualifications
•Current LPN licensure in the state of practice.
•Experience in behavioral health or social work is preferred.
•Knowledge of crisis intervention strategies and addiction counseling techniques.
•Strong supervisory skills with the ability to lead a team effectively.
•Proficient in crisis management practices within a healthcare setting.
•Familiarity with motivational interviewing methods is advantageous.
