As a qualified service provider with the Division of Developmental Disabilities (DDD), we provide physical therapy to individuals with intellectual and developmental disabilities (IDD) throughout the entire state. Our primary focus is on physical therapy habilitation to sustain functionality and promote a healthier lifestyle, regardless of their physical or intellectual limitation.

This is a part-time home care role for a Physical Therapist at Hometown Fitness & Wellness, LLC. The Physical Therapist will be responsible for providing patient care and habilitation services. This includes creating personalized treatment plans, evaluating patients' conditions, and monitoring their progress. The Physical Therapist will work closely with individuals in their homes to help improve their balance, flexibility, coordination, and overall physical fitness. The Physical Therapist has complete control over scheduling and treatment modalities of doctor prescribed therapy.

Qualifications:

Pay predicated on experience.
