Enrollment Case Manager (ECM) Location: Travel within assigned and neighboring counties required

We are looking for a dedicated Enrollment Case Manager (ECM) to assist individuals applying for community-based services. This field-based role requires conducting assessments and supporting applicants through the enrollment process.
