Job Summary
Taylor Diversion Programs, a residential independent living program for at-risk youth ages 14-21 and located in Sheffield, PA, is seeking an experienced Nurse: LPN/RN to assist in the care of all youth enrolled in their program. The nurse in this position will maintain standards of youth care and will collaborate regularly with colleagues in health matters related to youth at all levels. He/She will also assist in following the responsibilities and duties of this position.
Responsibilities and Duties
The successful candidate will:
•Administer all required exams/evaluations/observation during the intake procedure of a new incoming youth
•Record all information during the intake procedure, then follow-up with necessary care related to the outcome of the intake
•Schedule necessary medical and dental appointments for all youth; schedule follow-up as necessary.
•Administer medication throughout the youth population
•Provide certified "med training" to colleagues during orientation or on an as- needed basis
•Provide emergency care and treatment to youth
•Appropriately document and record all youth information
•Monitor health/medical progression of youth in carSupervise and support the youth population with medical needs and requirement
•Supervise and support the youth population with medical needs and requirements
•Assist in the development of nutritional program for breakfast, lunch and snacks
•Assist with prevention education

Case Management:
•Report any events requiring further investigation
•Complete and maintain all youth medical records
•Prepare youth reports and summaries
•Secure proper documentation for youth files meeting Federal and State requirements and organizational standards
•Facilitate communications on youth status with essential personnel
Qualifications and Skills
•3 years (minimum) preferred LPN/RN Nurse Experience
•Clinical experience, urgent care or emergency settings preferred
•Current LPN/RN Nurse License
•Current CPR Certification
•Current Tuberculosis Test and Drug Screen
•Reliable transportation to all scheduled appointments
Required Clearances:
•PA Child Abuse and PA State Police
