Hospital in Your Home (HiYH) is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our growing community-based care team in the Metro Atlanta area, including Dekalb, Fulton, and Clayton Counties. Hospital in Your Home (HiYH) provides Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and community-based clinical services to patients throughout Metro Atlanta and surrounding areas. Our mission is to improve health outcomes by meeting patients where they are—literally—while empowering them to manage their health from the comfort of their homes.
As a Community-Based LPN, you will work autonomously in the field while staying closely connected to our clinical leadership team. Responsibilities include:
- Conducting structured wellness and clinical assessments in patients’ homes
- Collecting and documenting vital signs (BP, pulse, etc.)
- Performing medication reconciliation and basic physical assessments
- Accurately documenting medical, social, and family history in the EMR
- Providing patient education to promote engagement, adherence, and self-management
- Ensuring timely, thorough, and accurate clinical documentation
- Communicating visit outcomes and follow-up needs with the Director of Nursing
- Supporting Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) workflows
- Representing HiYH at community health and wellness events
- Delivering compassionate, culturally sensitive care in home and telehealth settings
- Maintaining full compliance with HIPAA and applicable healthcare regulations
We are intentionally seeking experienced LPNs who bring professionalism, knowledge, and strong time-management skills to their work.
- Active Georgia LPN license
- Minimum of 5 years of clinical experience (care coordination or case management experience strongly preferred)
- Excellent organization, documentation, and time-management skills
- Strong interpersonal and communication skills
- High level of integrity, honesty, and professionalism
- Comfort working independently in the community
- Proficiency with EMRs and healthcare technology
- Reliable transportation and willingness to commute throughout Metro Atlanta and surrounding areas including Dekalb, Fulton, and Clayton Counties
- Schedule: Monday–Friday, 9:00 AM–5:00 PM (non-flexible)
- Compensation: Competitive pay plus mileage reimbursement
- Reporting To: Director of Nursing
- Work Environment: Patient homes, community settings, and telehealth
